PROVO, UTAH (July 13, 2012) - - Birth and death certificates, immunizations, and environmental health and food service permits will all be available starting Monday, July 16th at the Utah County Health Department's new American Fork office.
"With all the growth in the northern parts of Utah County, we needed full time officers closer to the homes and businesses of citizens in those cities," said Dr. Joseph Miner, Utah County Health (UCHD) executive director. "Birth and death certificates and immunizations are now available every business day full time in our American Fork offices."
The new office is located on the north east corner of 600 South and 500 East, just east of I-15 exit 276, in American Fork. "We are in the same building as our American Fork WIC Clinic," said Dr. Miner. "The location is very convenient off of the freeway."

"Health Promotion classes will be held at the American Fork offices in the evenings," said Dr. Miner." It will be wonderful to be able to offer such activities as healthy lifestyle classes, car seat installation classes, and our Living Well with Chronic Disease class."
